communication with other health care providers involved in your treatment.
Payment: To obtain payment for services provided, including billing you, your health plan, or other
responsible parties.

staff training, business management, licensing, accreditation, and legal compliance.
We may also disclose your information when permitted or required by law, including for public
health activities, health oversight, workers' compensation claims, law enforcement requests, court
orders, or to prevent a serious threat to health or safety.
Uses Requiring Your Authorization
We will obtain your written authorization before using or disclosing your health information for
purposes not otherwise permitted by law. You may revoke your authorization at any time in writing,
except to the extent action has already been taken.

health information; request restrictions on certain uses or disclosures; request confidential
communications by alternative means or locations; receive an accounting of certain disclosures;
obtain a paper or electronic copy of this Notice; and file a complaint if you believe your privacy
rights have been violated.

Evertide Pelvic Health is required by law to maintain the privacy and security of your protected
health information, provide you with this Notice, follow the terms of this Notice, and notify you if a
breach of unsecured protected health information occurs when required by law.
